Facility Use Request Form (FURF) and guidelines
Scheduling Parish Spaces for Ministry and Events
We are grateful for the many ministries and groups that bring life to our parish. To ensure our spaces are used effectively and respectfully, all meetings and events must be scheduled through our Facility Use Request process.
The Facility Use Request (FURF) system helps us coordinate space, support your needs, and care for our parish facilities with intention and stewardship.
- All requests must be submitted in advance
- Requestor must be 18+ and a registered parishioner
- Private events require proof of insurance (if non-parish sponsored)
Call or email Maureen Jarrell for the form and online link - Assigned spaces must be respected
- Setup/cleanup details are required
Please complete the online Facility Use Request Form (FURF) below or you may download and complete the FURF pdf and return to Maureen Jarrell at mjarrell@ascensioncatholic.net or drop off at the office.

This information will clarify facility use responsibilities and help us support you in planning and hosting your ministry events.
IMPORTANT NOTE: The application of tape and adhesive products is not permitted on parish facility floors and walls. Nails, tacks or other materials that may leave a permanent mark are prohibited. Please do not drag tables, chairs, furniture, etc. across floors as this may result in damage requiring maintenance services. Damage to facilities (or cleanup required by maintenance team not previously agreed upon) may compromise future use of parish facilities.
